First cross posting is not needed nor wanted!
Is PI4 still powering with microUSB ? IF so it would be most likely a bad contact there.
I Don't think your power supply is failing 5,25V is inside tolerances , where did you measure?
Can you measure the voltage and possibly the ripple on the IO header ?
Can you try another cable and or power supply?
( I am powering my PI3 directly to the IO Header to prevent problems with the tiny connectors.)

I have deleted the other post. Reproduced here:

Urgent!
Power supply failing. Production date: P4719 (Week: 47, Year: 2019)

Raspberry Pi 4 power supply voltage at 5.25 Volts while the Pi 4 is up and running.
Top right hand corner of the screen shows a yellow lightning symbol.

Model: KSA-15E-051300HE 100-240V Made In Cambodia

It was working fine till yesterday. I used it from Feb. 2020 onward.

What operating system are you using?

That voltage output is within normal limits.

So, what is making you think you have a voltage issue?
Pi 4 will not properly power up?
What?

Power limits for the Pi 4
https://raspberrypi.stackexchange.com/q ... quirements

If this is an actual official Raspberry power supply.
Load regulation: ±5%
Line regulation: ±2%

So, 5.25V could be regulated voltage based on what is going into it and demand being put on the power supply.

What voltage reading are you getting at the wall outlet?

The things you do not tell us, are usually the clue to fixing the problem.
When I was a kid, I wanted to be older.
This is not what I expected :o
