Hi Keef,
Which version of Puppy Linux and Nethogs are you referring to? I am running FossaPup64 and the nethogs_0.8.5-2build2_amd64.deb in the Puppy Package Manager does not have "/usr/usr"

yad [ $? -ne 0 ] && exit
echo OK was clicked
Usually OK returns 0 and Cancel returns 1. Not sure why it reverses with your example. maybe yad and the [ ] sections are being evaluated instead of just [ ].
For your example, the code can be shortened to
yad && exit
echo OK was clicked

I took a look at the ISO you posted. Looks pretty good and clean. I especially like the extra wallpapers.
I think you are missing the fdrv_fossapup64_9.5.sfs file. That might also explain why I was having network setup issues.
Earlier, I created a similar ISO. My ISO is ...

Re: YAD capture button click
continuing on with what wizard said, there is another way to interact with buttons. You can run actions, such as call a function, when a button is clicked. However, wizard's methods of interaction are more commonly used.
As an FYI, I have made new YAD documentation in Notecase format ...
